There are three different ways to send in your monthly reports: By email, fax or by mail. To report by email, simply select your officer from the list below, fill out the report form and click submit. The program will email your officer your report in the form of a pdf and will also email you a copy of the submitted report for your records. Please be sure to enter in your email address correctly. This form is available in both English and Spanish. The alternative options are to simply print out of the form and then fax it to our office at (239) 774-6143 or mail it to your officer at Collier County Probation, 3315 Tamiami Trail East, Suite #101, Naples FL 34112. To print the form click the print icon or use the keyboard combination of (CTRL-P).

Must be paid by cash, cashiers check or money order at the courthouse. All payments received by mail must be in the form of a money order or cashiers check. Please place your case number on all your payments . Probation Fees and Court costs must be paid through separate cashiers checks or money orders.


Restitution for cases that are being monitored through probation are payable through the probation department. All restitution payments must be made by money order or cashiers check, payable to the victim in the case. Please place your case number on all restitution payments.

In order to complete court ordered community service you must first pay the insurance fee. The cost of the insurance is $5.00 for every fifty (50) hours of community service you are required to work. Payment may be made in person or through the mail by cashier’s check or money order payable to Collier County Clerk of Courts. The memo section MUST reflect your case number and that the payment is for COMMUNITY SERVICE INSURANCE. Once the payment is received, a log sheet will be mailed to you. The supervisor of your community service location will need to record the hours and sign off on the form each day that you work. The name of agency, supervisor name, a telephone number for the supervisor, and the tax exempt ID number must be provided to our department in order to receive credit for the hours. Community Service hours have to be completed at a non-profit organization, tax - supported or tax exempt identity. Once probation receives the completed form the community service coordinator will contact the agency to confirm the authenticity of the form.
